Hur byter man vård central?

Det finns tre olika allternativ att byta vårdcentral.

Första alternativet, via nätet 

Så här gör du: 

steg 1. Trycker på ”Välj oss” på vår sida https://vardcentralhjartat.se/.

Steg 2. Scrollar ner tills du ser rubriken ”Gör ditt val direkt på nätet”

Steg 3. Tryck på den blåa knappen ”gå till 1177.se”

Steg 4. Logga sedan in på 1177.se med hjälp av din personliga bank-id.

Steg 5. Tryck på ”välj eller ändra”.

Steg 6. Välj vårdcentral och godkänn valet.

Alternativ 2, via post

steg 1. Tryck på ”Välj oss” på våran hemsida https://vardcentralhjartat.se/.

Steg 2. Scrolla ner i sidan fram tills du ser rubriken ”Ladda ner & fyll i blankett”, tryck för att ladda ner och skriv ut. 

Steg 3. Fyll i blanketten och skicka den till oss via post till adress: Bergavägen 1A, 254 66 Helsingborg.

How to change Health Center?

You have three different alternativs on how to change your health center.

The First alternativ, Is through the web.

This is how you do it:

Step 1. Press ”Välj oss” (means ”choose us”) on our website https://vardcentralhjartat.se/.

Step 2. Scroll down until you see the headline ”Gör ditt val direkt på nätet” (means ”Make your decision through the web”).

Step 3. Press the blue button ”gå till 1177.se”(means ”go to 1177.se”).

Step 4. Then press the button ”logga in”(which means ”log in”), then there will be different alternatives on how you want to log in to your account.Choose the one you want and sign in.

Step 5. Press ”välj eller ändra” (which means ”choose or change”).

step 6. You will see two tabs. Press on the first tab hälso- och vårdval (which means ”health and care choice”) choose ”VårdCentral” then press on the second tab område (which means city) then scroll down until you see Helsingborg choose it. Then Press button ”Sök”(means ”Search”).

Step7. Scroll down until you find ”VårdCentral Hjärtat” press it. Then accept agreement then press on the blue button.

Alternativ 2, Through the mail

Step 1. Press on ”välj oss” (means ”choose us”) on our website https://vardcentralhjartat.se/.

Step 2. Scroll down till you find the headline ””Ladda ner & fyll i blankett” (which means ”Download & fill form), press it to download the form then print it out.

Step 3. Write your information on the paper then mail it to us at the

adress: Bergavägen 1A, 254 66 Helsingborg.
